Insurance
Dr. Provenzano no longer participates as a preferred provider in any insurance panel. Direct payment from clients is required at the time of service or at the time appointments are scheduled. We will provide you with documentation needed to pursue any out-of-network coverage that might be available to you from your insurance provider, but it is your responsibility to seek it directly from that insurance provider.
Gifted Assessments
IQ and achievement testing required for school admission/gifted programs is not covered by insurance and therefor payment is due at time of service.
Scheduling Fee
Gifted Assessments require a $100.00 scheduling fee at the time that the first appointment is scheduled. Comprehensive Evaluations require a $250.00 scheduling fee. This fee is non-refundable and is credited toward payment of the scheduled service. However, it is forfeited in the case of missed, cancelled or rescheduled appointments.
